Mission

The mission of Able Palms Home & Health Care Services, Inc. is to support independence and enrich the quality of life of our clients by providing high quality, caring, personalized service, enabling clients to achieve an optimum level of wellbeing.

Purpose

Provide personal care services under the direction of the RN, Physical Therapist, Occupational Therapist or Speech Therapist. The Home Health Aide is assigned to specific clients and performs services for clients as necessary to maintain their personal comfort and as per the plan of care.

Essential Job Functions Duties and Responsibilities

  1. Perform personal care activities in accordance with state regulation and as per specific client care instructions and the plan of care, including but not limited to bathing, shampooing, skin care/nail care, oral hygiene, shaving and dressing.
  2. Perform household services essential to health care at home in accordance with state regulations, including but not limited to meal preparation/feeding, laundry, light housekeeping.
  3. Perform simple procedures as an extension of therapy services under the direction and supervision of the therapist. For example, range of motion exercises, assistance in ambulation or exercises.
  4. Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse, Therapist or Director of Clinical Services.Follow reporting guidelines in the plan of care.
  5. Promote personal safety and a safe environment for clients by observing infection control practices following Agency guidelines, and reporting unsafe situations to the Registered Nurse/Therapist or Director of Clinical Services.
  6. Document care provided on the correct form and according to agency procedure.
  7. Communicate effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
  8. Demonstrate safe practice in the use of equipment.Notify supervisor of educational/orientationneeds prior to using equipment.
  9. Provide nutritional support as needed.
  10. Attend in-service programs to meet compliance requirements. Identify learning needs to supervisor.
  11. Maintain confidentiality in all aspects of the job.
  12. Perform other related duties and responsibilities as deemed necessary.

 

Required Knowledge, Skills and Abilities

  • Must be at least eighteen (18) years of age.
  • Demonstrated ability to read, write, and follow a written Plan of Care.
  • Must possess the ability to make independent decisions, follow instructions, think critically, and accept constructive criticism.
  • Must be able to work with ill, disabled, elderly, emotionally upset, and potentially hostile people.
  • Must be able to deal tactfully with clients, staff, vendors, and the general public.
  • Good verbal, written, and interpersonal skills.
  • Must be able to speak and write English in a manner that is sufficient for effective communication.
  • Must receive criminal clearance and other required screening prior to employment with the agency.
  • Must obtain required health screening and TB clearance (see also physical requirements below).
  • First Aid training (training may be provided upon hire).
  • Current Red Cross certified CPR (training may be provided upon hire).
  • Valid and current drivers' license with appropriate endorsements.
  • Must be able to travel locally, including adequate automobile insured in accordance with applicable state law.

Education and Experience

  • Successful completion of a formal certification training program, including classroom and supervised practical training (minimum of 75 hours) and/or successful completion of competency exam. An individual is not considered to have completed the required training and competency evaluation program if, since the last such program, the individual has gone a continuous period of 24 consecutive months without furnishing covered services.
  • Minimum of six (6) months of work experience in a supervised setting, preferably a health care facility.

 

Licensing/Certifications

Must have current certification as a Home Health Aide in the state of practice, where applicable, and meet applicable training and/or competency evaluation requirements.
